Where a bio-link QR code actually earns its place
A QR code only helps where someone cannot tap. On screen they will always tap, so a code there is decoration. Print and physical space are where it does work.
Print, packaging and merch
Flyers, stickers on an order, a table card at a market. Anything a person holds. Print it at least two centimetres across and test it from the distance people will actually stand.
Slides and video
A code on a slide or in a video frame is the only way to hand a link to a room. Leave it on screen for at least five seconds and keep it clear of the lower-third captions.
Another screen, not the same one
A code on a laptop scanned by a phone works. A code on the phone the viewer is already holding does not. Use a plain link there.
What breaks a QR code, and how to avoid it
Most codes that fail in the wild fail for the same handful of reasons, and all of them are decided before you print.
Printing from the on-screen image

Inverting the colours
Scanners expect a dark pattern on a light background. A light code on a dark card looks better and reads worse, and some phone cameras will not attempt it at all.
Crowding the edges
A code needs a quiet margin around it. Butting artwork against the pattern is the single most common reason a printed code fails.
Routing through a link that can expire
Once a code is printed you cannot change where it points. Send it to a page you control and can edit later, not to a campaign URL or a temporary shortener.

No. The code is a direct encoding of the link you typed, with nothing of ours in the middle, so there is no account to lapse and no redirect to switch off. It works for as long as the link you pointed it at works.

Not with this tool, because it adds nothing to your link. If you need scan counts, point the code at a page that has its own analytics. A free Links page records every click on every destination, so the QR sends people to a page that can already tell you what they did next.
About two centimetres across for something held in the hand, and roughly a tenth of the scanning distance for anything further away, so a code read from two metres wants to be about twenty centimetres. Always test a real print before ordering a run.
Usually one of four things: it was printed from the small on-screen preview instead of the download, the colours were inverted, artwork was pushed against its edges, or it is too small for the distance. Reprint from the 1024 pixel download, keep it dark on light, and leave a clear margin.
